A manhunt for a missing patient from the Hawaii State Hospital in Kaneohe with a violent past has expanded to Maui, and a city prosecutor warns that the man is still a danger decades after his most notorious deadly assault.

Hawaii's Gov. David Ige said the public and authorities should have been made aware of the incident sooner, and said the state has started a review of the hospital's practices.

The assessment by Dr. Gene Altman said three of the relationships were reportedly with women in the community, including Saito's first and second wives.

Seven hospital staff members were being placed on unpaid leave Wednesday for 30 days and more may be identified as the investigation continues, the department said in a statement. She said the man fit the description of Saito.

Saito's victim was selected at random, authorities said at the time, shot in the face with a pellet gun and then fatally stabbed.

Saito was acquitted by reason of insanity for the gruesome 1979 killing of 29-year-old Sandra Yamashiro at Ala Moana Center.

FOX40 spoke to a manager at the motel, he said the San Joaquin County Sheriff's Department spoke to them and confirmed Saito was never a guest at that Motel 6 location.

Saito was considered "extremely unsafe and should not be approached", the Honolulu Police Department warned.

Late Tuesday night, November 14th, the Hawaii Attorney General's office charged Saito with felony escape and issued a $500,000 bench warrant for his arrest.

Honolulu police say Randall Saito left the Hawaii State Hospital at 9 a.m. Sunday and failed to return. "He's a very unsafe individual with respect to whom all the predictors indicate that if he were to be released, he would kill again".

He paid $1,500 for the chartered plane ticket, according to Maui Now. Police wouldn't provide details about his flight to California. Hospital staff called 911 to report his disappearance shortly after 7:30 p.m. - two hours after he landed in San Jose, police said.

Hawaii State Hospital Administrator William May said officials are fully cooperating with law enforcement and appropriate steps would be taken if Saito had help from someone inside the facility.

In 1981, his attorney did not contest Saito's committal, saying he was risky and "intends to do it again".
